Why form numbers come first

## A packet can contain several different legal events.

A divorce Petition, a hearing request, a financial declaration and a proof of service can arrive together. Treating them as one undifferentiated pile hides which questions and dates belong to which event.

Can connect **Common packet and workflow patterns**

Cannot decide **Validity, truth, filing, service or legal strategy**

## Form reference

Read the form descriptions below, or use the chooser above to compare your papers.

### FL-100

**Petition—Marriage/Domestic Partnership**

Starts a divorce, legal-separation or annulment case and identifies issues the petitioner wants included in the judgment.

It records requests and allegations. It is not a judgment and does not prove those requests were granted.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-100) · [CourtAhead guide](https://courtahead.com/resources/fl-100)

### FL-110

**Summons (Family Law)**

Says a family-law case has started, describes response consequences and contains rules that can apply during the case.

The form itself does not prove when or how you were served. Service facts must be checked separately.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-110) · [CourtAhead guide](https://courtahead.com/resources/fl-110)

### FL-105

**Declaration Under UCCJEA**

Gives the court information about where children have lived and whether other custody cases may involve them.

It reports custody-jurisdiction facts. It is not a custody order or parenting plan.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-105)

### FL-115

**Proof of Service of Summons**

Tells the court who served the papers that started the case, along with the reported method, place and date.

A blank form proves nothing. A completed form reports service; this tool does not decide whether service was legally valid.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-115)

### FL-120

**Response—Marriage/Domestic Partnership**

Lets the respondent participate and describe the orders they want the court to make in the divorce case.

A copy in a packet may be blank. Its presence does not prove a Response was completed, filed, accepted or served.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-120) · [CourtAhead guide](https://courtahead.com/resources/fl-120)

### FL-140

**Declaration of Disclosure**

Lists the financial forms and information being given to a spouse or domestic partner as a preliminary or final disclosure.

It identifies the disclosure packet. A signed copy alone does not prove the packet was complete, exchanged or filed.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-140) · [CourtAhead guide](https://courtahead.com/resources/california-divorce-financial-disclosures)

### FL-141

**Declaration Regarding Service of Declaration of Disclosure and Income and Expense Declaration**

Tells the court which preliminary or final financial disclosures were served, how they were delivered and the reported service date.

It reports a service event. It is not the underlying financial packet or independent proof that every disclosure was complete or accurate.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-141) · [CourtAhead guide](https://courtahead.com/resources/california-divorce-financial-disclosures)

### FL-142

**Schedule of Assets and Debts**

Lists known property and debt and can accompany FL-140 as part of a financial disclosure packet.

It is party-supplied disclosure information, not a court finding about ownership, characterization, value or responsibility.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-142) · [CourtAhead guide](https://courtahead.com/resources/fl-142)

### FL-150

**Income and Expense Declaration**

Gives financial information used when the court considers support, attorney fees and certain other costs.

It is a party's signed financial disclosure, not an independent verification that every number is complete or accurate.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-150) · [CourtAhead guide](https://courtahead.com/resources/fl-150)

### FL-160

**Property Declaration**

Lists property and debts and can be used instead of FL-142 in a disclosure packet or in other property-related court filings.

The form records a party's position; it does not prove how property or debt is legally characterized, valued or divided.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-160) · [CourtAhead guide](https://courtahead.com/resources/fl-160)

### FL-300

**Request for Order**

Asks a judge to decide specific family-law issues and is also used to set the hearing date, time and location.

It shows what was requested. It does not prove the judge granted the request; check for signed orders separately.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-300) · [CourtAhead guide](https://courtahead.com/resources/fl-300)

### FL-320

**Responsive Declaration to Request for Order**

Tells the court whether a party agrees or disagrees with the orders requested on FL-300 and what they want instead.

It records a party's position and supporting facts. It is not the court's ruling.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-320) · [CourtAhead guide](https://courtahead.com/resources/fl-320)

### FL-335

**Proof of Service by Mail**

Reports which family-law papers another adult mailed, as well as when, where and by whom they were mailed.

It supports the service event it describes; it does not prove filing acceptance or the truth of the papers served.

[Official form](https://selfhelp.courts.ca.gov/jcc-form/FL-335)
